Clinical Labs WA are pleased to support your Telehealth consultations to make sure these patients can access pathology services to complement your care.
One of our friendly team will contact your patient, direct them to their most convenient location and ensure the form is available on their arrival.
Option 1: Physical referral forms
If your practice is using the physical Clinical Labs pathology referral forms, this is how it works:
1. Complete your patient’s pathology request on a physical Clinical Labs referral form2. Scan and email the form to Clinical Labs telehealth.wa@clinicallabs.com.au
Option 2: Electronic referral forms
If you wish to use our electronic referral form for general pathology, here's how to do it:1. Download our electronic referral form from our website clinicallabs.com.au/telehealth2. Complete the form and email it directly to Clinical Labs telehealth.wa@clinicallabs.com.au
Option 3: Using your practice management system
Using Clinical Labs eOrders, check the telehealth consultation box and a copy of the request form will be automatically sent to our telehealth team.
Using Clinical Labs eOrders, create a Pathology Request with TELEHEALTH in the Clinical Details section and a copy of the request form will be automatically sent to our telehealth team.

If your patient is unable to attend a collection centre for medical reasons, we can organise a home visit collect.
*Please email home visit request forms to osblab.homevisits@clinicallabs.com.au.* Please note, we are unable to conduct home visits for patients suspected of, or self isolating due to COVID-19.
